I don't know if you're familiar with Toriko, but there is a character, Tommyrod, that has insects inside him and he releases the Parasite Emperor that he can't control when he loses. He uses his energy to keep the Parasite Emperor from hatching and uses its freezing ability while he has control of it. Maybe these guys have some control over something inside them for now but need to keep it in.

I looked up previous videos of this guy, it looks like his neck has that "inclination". They are not as extreme as this video, yet close in a way. I think this is a genuine video which has not been edited.

Now, I have never seen this in real life, nor anyone I asked. It must be a very special case, considering his position, he must be jewish of pure blood, which leads to a scary conclusion.
