It's common for the ear lobes to change shape after a face lift or two or three. I hate to be boring but I thinks that's what it is. It explains his increasingly bizarre facial expressions - stretched, taut skin on an old geezer like him makes him look like some kind of lizard person. They subjected him to multiple facelifts and hairplugs etc to make him look more capable looking, and pump him full of drugs to make him seem more coherent. It's sick and desperate and disgusting. Also the sound of the first gunshots is out of sync with when he raises his rifle, shots happen first while it's still down - does it stay out of sync? Is this just an artifact of the heavy editing the video has undergone?
