I have a personal story. At university I went to the student health center to see about a minor issue. (This is where I learned that even though students pay for it, and I paid extra for insurance to be seen there, the place was really for MMR vaccines (without paperwork of any sort), sexual health, administering rape kits and not much else.)
The clinician noticed I looked sad so asked me about it. I confided that I was experiencing a bout of depression and then that person asked me if I was looking for pills. This seemed odd to me because I had not undergone any sort of medical evaluation. I said that my sadness was due to life circumstances and probably not related to an underlying chemical imbalance.
It was then that I was given a form like you described. I made the decision to reach out for help. Someone close to me was battling drug addiction and had just been given serious jail time and fines for it. One of the questions was worded something like, "Are you or is someone you know having a substance abuse issue?" I answered yes and it went on to ask which substance. I wrote down a particularly hard drug.
I naively thought they would refer me to a support group or at least give me some information. They had absolutely zero interest in helping me. The following week, the demeaner of two of my professors toward me became noticeably negative. A few days later, someone in another department who was previously quite amicable literally glared at me when I came to the office. It was a minor administrative task concerning my upcoming graduation. The person seemed so angry at me during the entire process; I wondered what would prompt such unprofessional behavior.
Then it clicked. Just a couple of years previously, I was asked to help sort surveys for the school of social work. Nothing was done to sanitize the data and I realized that I was literally seeing the sexual histories of people I recognized.
I believe the form was used to place me on a watch list given to university faculty of people at risk of going on a shooting spree or pushing drugs onto other students. These people were assuming I was a drug addict because of my answers on the psychological evaluation form.
