I'm not familiar with this dude's history in particular, but I personally have experience with a large LE agency, and that's how things are done. It looks like he never made the rank of commander but was the acting commander of the Vegas shooting. It's also the cause of many pissing matches when the fire chief shows up to an arson, or when the FBI/ATF/USMS show up to something major because they don't want to deal with the shift lieutenant in charge, they want the Sheriff or Police Chief.
