I just recently talked to a few friends about this phenomenon. I honestly think upwards 85% of people have little to no internal monolog. I don't know how to describe it other than they are less sentient than those who do think in words. It was a frightening revelation but it explains why most people behave like cattle.
I think part of the reason our internal monologs may be more intense is similar to when you try to sedate someone against their will. The person gets a burst of adrenaline and becomes more awake than ever to respond to the threat. Its the same with us, we see how they are trying to sedate, hypnotize, program us, so our minds are waking up more than ever in response.
For those who may have some trouble imagining the internal state of the NPC, or even believing it is possible other humans live and function in such a way, almost all of us have actually personally experienced it.
Have you ever been on a long, boring drive, typically a commute to or from work, that you've done hundreds or thousands of times, and suddenly you're at or near your destination and realize you have no recall of the trip? It's essentially black. You could not name a single thing that happened or that you saw. You just "came to".
But think: you were operating a motor vehicle on a roadway, which is undeniably a complicated and demanding task. Tens of thousands of people every year are killed because of relatively minor errors during this activity.
That's what it's like full-time for NPCs. They live complex lives filled with all kinds of activities, but "inside", just blackness.
Yeah, this is totally different, you'd know it if it had happened to you. It's only happened to me a couple of times.
It's not like anything but the deepest sleep and really more like general anesthetic, if you've ever been put under that way. You're somewhere, then in the next instant you're somewhere else. No sense of time passing, movement, dreams, nothing.
I've heard something about some types of meditation that are aimed at this state of oblivion, but if that's what they're going for they're either crazy or it's just another scam to get us not to think.

Usually when that happens I'm zoned out thinking about other things, either past or future and forget to focus on the present.
So - people zone out into non-thinking?
It sounds different than the meditative state that helps strengthen thought processes.

It's not even about the use of language. Dude in the picture just didn't think at all
At the very least, I would expect people to be silently cursing each other, especially in traffic.
