Someone close to me has been under some medically hard times as of late (not covid related), and I've been making sure that every time they get moved from one facility or team to another, that everyone is 100% aware of pre-existing medication schedules.

They always are, but I sleep better at night when I respond to every "He needs to be on this med" statement with "And you're sure there are no harmful drug interactions with his current meds?"
