Likely the same way "flu" kills today. The use of pesticides (particularly in agriculture).

Not sure pesticides we used 100 years ago at least not used worldwide when they say up to 500 million people caught the flu and how would they be able to cunt 100 or 500 million people anyway. Its a odd one for sure.
