The document and questions were intentionally designed as a simulation of McCarthyism and to make students question the appropriateness of the information being requested. In fact, within a minute, the teacher explained the purpose of the exercise was not to collect this information, but to demonstrate how private citizens’ rights and privacy were invaded during McCarthyism in the 1950s. The document which contained the questions was collected and destroyed.
In the future, if you should see something that doesn’t seem right to you, please reach out directly to your child’s teacher or South administration before sharing it on social media, where unfortunately it only causes misunderstanding and disruption to our teaching and learning environment.

'McCarthyism'? This is positioning kid's minds that combatting Communism is bad. It's subtle agitprop.
Considering how hard it is to determine if this is real or not, I'd assume it's an attempt to see what they can get away with.
If a kid, or parent, complains; hey, that was a test, and you passed.
If not, you do the next class.
