One of the biggest cover-ups for their failures is those stars they put on the wall to signify a death. If they publish the names of all the CIA employees who died, there would be outrage. Instead they put up a star up after a number of people die and claim it is just one person. This just represents CIA employees, there are countless more contractors, informants, and couriers who are killed with no remorse or accountability.
