Also, the various combatant commanders are the ones in charge of things like security (NORTHCOM specifically in this case) and they wouldn't have to report things like this for approval to some acting secretary.
As for Pelosi, it's come through various other sources and channels that she wanted a bigger mess than what actually happened. Is it she, and the capital police (again, who don't report to Trump) who are in charge of security.
