I have come to believe, it's a complicated and individual answer. There is CICO, genes(which do play a part), the rise in HFCS and other highly processed "foods" and gut microbiata. Your gut biomes plays a larger role than most acknowledge. CICO is the norm, it is baseline. But when a person spends a large portion of their formative years eating trash, it fucks up your gut biome and in turn that screws up CICO.
And fat people just Do Not move as much or as intensely as a fit person. They will absolutely do the least amount of physical work possible. Take the stairs? nope. Park farther out and walk? Nope
There's a lady at work. She's probably 4'10" or so and built like a pear. Enormous wide hips and butt. She eats lunch at the same time I do, so I see her lunch. It's always decent stuff she cooked at home herself, it isn't as a rule what I'd call goyslop. It isn't excess amounts, and I don't think she gorges at home. I've never seen her drink a soda. She will never be svelte. Her genetics aren't playing that game. But if she does any exercise I'll bet isn't intense or long. And her gut biome might be fucked up too.
All that said, fuck fat people. Most don't even try. They just make excuses and expect me to gladly surrender part of my airplane seat.
What's really weird to me is that I've known and worked with several very obese people and their relevant behavioral habits weren't actually different from everyone else's, it wasn't like they refused to move around as much as everyone else and their meals weren't unusually large nor ridiculous things compared to what everyone else was eating, the only way it made sense to me was if they were binge eating secretly when alone? Because I'm not talking about people being a bit chubby but rather full blown morbid obesity.
for the obese construction guys i've worked with, they don't seem to be living differently, at least on the job sites. they do tend to be more supervisory. worth noting, of the 7 or 8 OSHA safety orientations i've had this year at different sites, the OSHA enforcer has always been significantly over-weight if not obese.
I joke, but only partly. iirc, wolves can become totally domesticated in several generations. what are we doing to ourselves? what traits is society long-term self-selecting for?
If the medical industrial complex and the FDA were actually there to help people then this wouldn't exist. If we wish for things to change then refer to those two again and ask yourself how do we get past the poisoning they induce?

Lol, like they could catch a fish.
