I actually think the burl is being used to disguise the image. If you look around the body at the bottom right of the 'tree' you will see her legs do not come together with the trunk on the right. This suggests there is a picture of a tree which she has been shopped into and the perspective is different.

She is laying down on the floor or something. The tree picture is looking up.
