So this one happened in Japan in 2018, and they found tooth fragments in a McMuffin sausage patty. I suspect some tooth fragments of a pig made its way through the processing and found it's way into a patty. Pig anatomy is somewhat close to humans, and a small chunk of pig tooth could be mistaken for a human tooth.
