Real elites controlling the world most likely live phsyically outside the modern world. Likely some huge nice land unchecked on maps.

Their majordomes such as rothschild likely have their own ways to avoid pollution as in point 1. But they may be required to withstand a certain level of risk. Privileged workers, but still workers.
