It looks like he wore 2 shirts. A blue and gray one. Blue one has a front pocket with some stuff. That one is off when he layed down.

The shirt was clean with dirty patches, later it's all dirty and dusty, as you mentioned one of the image may have been mirrored or the stain could be less visible due to the entire shirt becoming dirty later or you know, the invention of beating your shirt to quickly remove dirt. He was killed after he marched a distance after the photo.
when Syria was in the public eye 2016 through 2017, the white helmets were caught multiple times staging dead people photos. I wouldn't be surprised if the same shit is going on here.
