What would happen is that whatever health system in place for ordinary people would get easily overwhelmed during any crisis. The superclass would be able to pay more for medical treatment if that were to happen. The implication is that medical staff have been deliberately targeted for elimination in the form of a shot known to cause debilitation or death in order to indirectly cull a large portion of the population from lack of needed medical care during a manufactured future calamity.
