I suppose a big problem in introducing nanotech into a human is that you can't install batteries. So there has to be a way to extract electrical power from the host. That could be done in electrochemical ways where a chemical reaction may use oxygen and fuel in blood to generate an electrical potential. Here, one could make energy harvesters at a micro scale.
